This Privacy Policy explains how Wellspring Counseling ("we," "us") collects, uses, and protects information through this website. A separate Notice of Privacy Practices (below) covers protected health information governed by HIPAA.

Information we collect on this site

  • Information you give us, name, email, and the contents of any message you send through the contact form or by email.
  • Assessment responses, if you take the Instinctual Drive Readout or 6 Sides of You assessment, your answers are stored to generate your results.
  • Basic site analytics, anonymized data about pages visited, browser, and device, used only to improve the site.

How we use it

To respond to your inquiry, deliver requested resources or assessment results, schedule appointments, and improve the site. We do not sell your information to anyone, ever.

How we protect it

Email and messages are stored in industry-standard, encrypted services. Health records are kept in a separate HIPAA-compliant electronic health record system, not on this website.

Cookies

This site uses minimal, functional cookies for navigation and anonymous analytics. No advertising trackers.

Your choices

You can request a copy of any information we hold about you, ask that it be corrected, or ask that it be deleted (where required records-retention rules allow). Email matt@6sides.org.

Uses and disclosures of PHI

Your PHI may be used and disclosed for treatment, payment, and healthcare operations. With your written authorization, it may be disclosed to others you specify. We are required by law to maintain the privacy of your PHI, provide you with this notice, and abide by its terms. We reserve the right to change the terms of this notice; the most current version will always be available here.
